Job summary

We are looking for an enthusiastic Community Primary Nurse to work i ourKingston Older Peoples CMHT. The Kingston Older Peoples CMHT, Memory Service & Care Home Team has been well-established for over 20 years. In the past year our services are being evaluated through transformation. During the new formation of the team many new roles have been developed. All our staff work across both the Memory Service, Older People’s CMHT & Care Home Team and we enjoy integrated caseload management, helping to better meet our service user’s needs and patient safety.

Main duties of the job

The role of Community Mental Health Nurse is both demanding and very rewarding. You will be responsible for reviewing service users and supporting carers and relatives which may be completely new to Mental Health Services. You will also be responsible for a caseload of service users with complex needs, some may have chronic and acute mental health needs. There will be an expectation to working independently and provide effective caseload management. Knowledge and working with old age mental health conditions would be an advantage.

Holistic assessment alongside a recovery style approach will be essential in supporting our service users. There will also be an expectation to attend interdisciplinary meetings, help with auditing and improving the team’s key performance indicators. Mandatory and developmental training will be provided. Having access to your own car and driving will be an essential requirement for the post holder to help them complete home visits. A mileage allowance will be provided.
