The MLCSU Personalised Healthcare Commissioning (PHC) Team based in Leicestershire are committed to ensuring excellent healthcare for all individuals.

Due to extra funding We are looking for a motivated and experienced Learning Disabilities (RNLD) ,nurse who can work autonomously and collaboratively in a multi-agency environment with individuals and their representatives to support the assessment and review process for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC). You will undertake comprehensive person-centred assessments to CHC and FNC eligibility.

You will have a good understanding of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care, excellent communication and problem-solving skills, have a proven ability to produce high quality documentation whilst working to deadlines and targets.
