We have an exciting opportunity for an experienced and innovative Advanced nurse practitioner to join the team South Meirionnydd Community Resource Team funded by the Meirionnydd Cluster

The cluster has proposed the development role of a Community Resource Team Advanced Nurse/Clinical Practitioner to support the cohort of housebound patients in South Meirionnydd, – by providing a rapid response to those in the greatest need alongside planned care in managing chronic and acutely unwell patients with highly complex and multiple co-morbidities and to recognise patients with high Frailty and initiate CGA and Anticipatory/ Advanced Care planning.

They will foster a culture of excellence and continuous improvement ensuring the highest standard of nursing care is provided, monitored and evaluated.

Main duties of the job

Key aspects of the role will include:

Working as an autonomous, advanced practitioner, with an extended scope of practice to work with the team to deliver care; providing a high standard of specialist assessment and treatment of patients. Using evidence-based principles/ current best practice and advanced specialist skills and clinical reasoning to assess, plan, implement, and evaluate interventions.

Ensuring all clinical practice is based on current research /evidence, developing and undertaking audit and research as appropriate. Participate in quality improvement initiatives, Clincal audits and research projects to enhance patient outcomes.

Demonstrating through day-to-day action a contemporary approach to Primary & community Care which is patient-centred, prudent and team-enabling.

They will maintain a visible professional presence where patients, carers and staff can turn for assistance, advice and support.

They will lead and participate in projects and areas of work, which will reflect clinical, professional and cooperate objectives.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
