Job summary
The purpose of the Breast Cancer Patient pathway navigator, supporting SFU is to support the Personalised Stratified Follow-Up (PSFU) Clinical Nurse Specialists (CNSs) to deliver safe effective follow up care to patients who have had treatment for breast cancer. The role is responsible for overseeing the transfer of patients from the Somerset Cancer Registry database (SCR) to the Somerset Remote Monitoring System (RMS).
The role is responsible for functions within and related to the RMS including escalation of variances, generating letters and appointments. They will support the provision of Personalised Cancer Care to patients on the PSFU pathway in conjunction with the Breast PSFU Clinical Nurse Specialists. They will coordinate the surveillance programme and ensure that relevant letters are sent out to the patients and their GP. They also ensure that due tests are escalated to the CNSs. They will assist with collecting and collating data about the service to assist with monitoring and improving it and ensuring patients benefit from Supported Self-Management interventions.
Main duties of the job
The right candidate for this role will be keen and experience administrator within the NHS with ambition to learn clinical admin to support the nurse function withing long term Breast Cancer service.
A key aspect of the role involves daily and direct (e.g. face to face and telephone) communication with breast cancer patients and other health and social care professionals.
The role includes undertaking patient-facing duties including aspects of Holistic Needs Assessments and End of Treatment Summaries in conjunction with the Breast PSFU CNSs. The information and nature of the communication required is sensitive due to the nature of cancer. Communication in this context requires a high degree of empathy, understanding, diplomacy, honesty and integrity.
