Job summary
We are looking for an experienced renal nurse to become part of this highly skilled team. You will oversee the delivery of high quality, safe, effective care focused on improving outcomes and experiences for patients, families and carers.
The Renal Clinic at Hammersmith Hospital provides ongoing outpatient care for our transplant, Vasculitis, Lupus patients. We have combined cardiac, rheumatology, ENT and Eye clinic.
We have 4 inpatient wards totaling 71 beds (12 HDU) and a programmed investigation unit offering a ward attender/day case service of 18 beds.The service also has an acute haemodialysis unit, 9 satellite dialysis units, peritoneal dialysis and home haemodialysis teams; nurse led chronic kidney disease team, Live donor team and 2 outpatient clinics looking after over 3,100 patients on renal replacement therapy.
Main duties of the job
- Act as a clinical expert within your sphere of work.
- Carry out planned care for a group of patients without direct supervision.
- Ensure the assessment, planning, implementation and evaluation of evidence based, individualised patient care as appropriate within an outpatient setting.
- To be responsible for reception and appointment functions within the department, ensuring that systems are in place in order that patients are seen in a timely manner.
- Plan, organise and deploy staff according to ability and workload in order to meet patient needs.
- Ensure that nursing practices comply with Trust policies and procedures.
- Always communicate effectively and maintain a harmonious working environment.
- Assess the needs of patients, relatives and carers and provide them with information as requested.
- To keep up to date with current developments in nursing and ensure evidence-based practice.
- Develop new skills and knowledge pertaining to the speciality as required.

- Teaching Qualification or equivalent experience.
- Relevant specialist course or equivalent experience
