Job summary

An exciting opportunity has arisen to work in a The Specialist Mental Health and Substance Misuse Housing Outreach Service, who support those in temporary accommodation or hostel in accessing the appropriate health services.

The aim is to deliver evidence based holistic mental health care to people within the context of an integrated multi-disciplinary, multi-agency specialist service.

We are looking for a creative practitioner with experience of working with and supporting complex individuals and advising accordingly on mental health and/or substance misuse related issues.

The team currently consists of Health, Local Authority and Third Sector staff. Including Trauma Informed Counsellors, Nursing staff, Social Workers and Support Workers.

The service runs Monday to Friday 9am to 5pm and covers all of CTM. Due to the expansion of the service our aim is to look at locality based working, this would need to be flexible to cover the needs of the service i.e. absence or where a high level of referrals are received.

Main duties of the job

The post holder will provide support and intervention through holistic person centred assessment, treatment and evaluation. Providing a proactive/outreach approach. They will demonstrate safe, clinical decision-making and expertise to support people within the community whether this be at home or temporary accommodation. They will take responsibility for a caseload of clients as referred to the service, provide specialist advice to external agencies and carry out appropriate teaching where necessary.

The post holder will work collaboratively with the wider multi-disciplinary team . This will generally involve working with adults who have health needs due to complications of drug, alcohol use and/or mental ill health and would support transitional arrangements for people requiring primary and secondary health support. As a relatively new service, the post holder will be required to contribute to the continuing promotion and development of the service through networking whilst representing the service at all times.
