We are looking for adedicated, professional and compassionate Practice Nurseto join our friendly community GP practice team.
This is an excellent opportunity for a motivated nurse to play a vital role in providing high-quality, patient-centred care to our local population. You will work closely with our GPs, healthcare assistants, and administrative staff to deliver a wide range of nursing services, including long-term condition management, health promotion, immunisations, and wound care.
The ideal candidate will have current NMC registration, be qualified inphlebotomy and injections, and have experience working withinprimary careand as part of a nursing team. You should also be comfortable working independently and providing care forvulnerable patientsin a community setting.

Main duties of the job
- Clinical Responsibilities
- Deliver evidence-based nursing care in line with clinical guidelines and NMC standards.
- Undertakechronic disease management, including reviews and education for patients with diabetes, asthma, COPD, and hypertension.
- Performphlebotomy,immunisations (including children’s), andinjectionssafely and efficiently.
- Providewound management,ECGs,blood pressure monitoring, and other clinical assessments as required.
- Conductcervical cytology.
- Support and delivervaccination and screening programmesin accordance with practice and NHS guidance.
- Promote health education, disease prevention, and self-care among patients.
- Identify, assess, and supportvulnerable patients, ensuring appropriate safeguarding procedures are followed.
- Maintain accurate, timely, and confidential patient records using clinical systems (e.g., EMIS or Vision).
- Participate in infection prevention and control procedures, equipment checks, and stock management.
- Work collaboratively with GPs, healthcare assistants, and administrative colleagues to ensure the smooth running of the practice.
- Support the practice in deliveringQOF,QI, and other quality frameworks.
- Contribute to audits, significant event reviews, and quality improvement initiatives.
- Participate in clinical meetings, training sessions, and supervision.
- Maintain professional registration and keep up to date with current evidence, guidance, and best practice.

Qualifications
Essential
- Registered Nurse with current NMC registration
- Qualified to NVQ Level 5 in Nursing
- Qualified in Injections / Immunisations
Desirable
- Cervical Cytology qualification
- Training in Chronic Disease Management (e.g., diabetes, asthma, COPD)
- Evidence of ongoing Continuing Professional Development (CPD) relevant to primary care
