We are seeking an experienced Practice Facilitator to help steer, drive, support and deliver service improvement within the Mental Health, Learning Disability and Neurodiversity (MHLDN) & Wellbeing Services directorate in Livewell Southwest.

The Practice Facilitator role is linked directly to the development of clinical and leadership

competencies for staff groups across, but not limited to the Community Nursing teams.

The Practice Facilitator role be involved in design, implementation, and delivery of training.

This will include classroom-based teaching, simulation and situational learning.

This person will have all the clinical competencies in place to be able to develop the skill and knowledge of the staff groups. The post holder provides support to the clinical nursing staff to underpin the principles of evidence-based practice and quality in professional practice development.

The post holder will be required to have excellent, effective, communication skills to

support staff development and enhance quality care.

Be able to use a range of teaching styles to meet the needs of learners with consideration

to neuro diversity and reasonable adjustment to ensure equity and accessibility for all.

Maintain and foster good relationships with professionals and non-professional colleagues

concerned with the provision or development of healthcare services.

The post holder will be required to use a computer, being responsible for timely recording

patient activities for IT data collection, dealing with e-mail queries, action development

plans.

Lead and participate in team activities so as to develop and consolidate a cohesive and

supportive team ensuring openness within the team.

You will be able to communicate complex, sensitive or contentious information. This may

be because agreement or cooperation is required or because there are barriers to

understanding. The Post Holder to be supplied with a mobile phone and for work purposes

and be expected to abide by the organisation policy.

Develop and maintain effective and appropriate communication networks with other

members of the Primary Health Care Team and other agencies.

Reporting and recording of all incidents and near misses as per policy.

Have good communication skills to facilitate effective communication with staff, patients,

carers including sensitive and accurate information.

The post holder will require need to actively participate in effective information sharing of

development areas within clinical competencies undertaken by staff
