Job summary
The States of Guernsey are looking for a Staff Nurse to join the Community Nursing Team.
The Community Nursing Team provides care, support and treatments to service users within their own homes and care home settings.
Job responsibilities
Responsibility for Patient/Service User Care
To assess, develop, implement and evaluate care plans demonstrating high standards of evidence-based care.
To work autonomously, recognising changes in patients’/service users’ presentation which require intervention, knowing when to escalate to senior staff.
To guide, advise and direct other staff in the delivery of programmes of care.
To support patients/service users in undertaking activities of daily living within care plans
To undertake the role of health promoter and educator for patients/service users and their support networks.
To report all accidents and incidents as per policy and participate in risk management and Quality and Safety processes e.g SWARM.
To be accountable for ensuring that safeguards are in place for patients/service users and escalate any safety, safeguarding or risk concerns following the appropriate processes.
Communication
To build a therapeutic relationship with patients/service users and their support network, communicating with them in a way that respects their views and cognitive function.
To use a wide range of communication skills that can be adapted to meet the needs of complex and challenging situations.
To establish effective working relations with all members of the multi-disciplinary team, clearing relaying condition related information in a timely and effective manner.
Communicates to patients/service users and relatives compassionately, in a confidential and sensitive professional manner.
To acknowledge and respect language and cultural differences of patients/service users, their support networks and colleagues.
To provide clear, concise, and accurate electronic and paper records of the care given in accordance with agreed policy and standards.

To participate in the induction, orientation and development of new staff, agency workers, students and any voluntary workers.

Person Specification
Qualifications
Essential
- Minimum Qualification:
- Registered Nurse with the NMC.
- Diploma in relevant field.
- Possess a full and valid driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for individuals in line with the Prevention of Discrimination (Guernsey) Ordinance, 2022.
Desirable
- Key Criteria:
- Ability to demonstrate an understanding of HSCs CARE values.
- Ability to plan and prioritise own workload and that of others.
- Ability to work shifts across a 7-day week.
- Good communication skills, both written and verbal, with the ability to deal with complex and challenging situations in a calm and diplomatic manner.
- The ability to work autonomously and also as part of a multi-disciplinary team.
- The ability to supervise and support junior staff.
- Punctual and reliable
