Job summary
Become a part of a developing service at the forefront of community-based Continuing Healthcare, Palliative and End of Life Care in Carmarthenshire. We are looking for experienced and motivated Registered Nurses (RN/RMN) to undertake a innovative Band 6 Sister/Charge Nurse role that combines community outreach with acute hospital in-reach.
Working across community and acute settings, you will play a pivotal role in early identification, assessment, care coordination, discharge planning and clinical decision-making, helping people receive the right care, in the right place, at the right time.
We are looking for dynamic nurses with excellent clinical, leadership and communication skills who are passionate about delivering person-centred care and improving outcomes for people with complex, life-limiting conditions. You will provide senior clinical oversight to support safe delegation, governance, and workforce development, while working closely with multidisciplinary teams, families and partner organisations to deliver seamless care across care pathways.
This is an exciting opportunity to contribute to a developing seven-day service that supports people to remain in their preferred place of care, prevents unnecessary hospital admissions and enhances the experience of patients and families at the end of life.

This includes hospital in-reach activities to facilitate early identification, timely Continuing Healthcare and Fast Track processes, coordinated discharge planning and seamless transitions of care from hospital to home.
Within the community, you will provide senior clinical oversight, assessment and care coordination, supporting proactive care planning, complex decision-making and multidisciplinary working to help people remain in their preferred place of care wherever possible.
You will provide clinical leadership and supervision, promoting safe delegation, governance and workforce development, while supporting Nursing Support Workers and colleagues to deliver high-quality, person-centred care.
