Main duties of the job

The post holder will work as part of the Macmillan Specialist multi-disciplinary team comprising of Medics, Dietician, Occupational Therapist, Physiotherapist, Nurses and Speech and Language Therapist. The team aims to provide effective support rehabilitation and/or palliative care to patients with a life limiting condition requiring palliative intervention.

The post holder will assist other professionals in the implementation, delivery and evaluation of rehabilitation and education programmes to promote independence and encourage improved health, functional capabilities and quality of life.

The post holder will hold a small caseload under the supervision of the responsible clinician, supporting patients with life limiting illness to maximise their health and enable them to be cared for in their preferred place.

You’ll also need to demonstrate how you will meet the ability to be mobile within community.

Qualifications

  • foundation degree or equivelent qualification
  • Health Related qualificaation

  • working in complex/ palliative care situation
  • evidence of good communiication

  • Commuter Literate
