Job summary

Imperial healthcare (Hammersmith Hospital Site) are seeking experienced Band 6/7 nurse to work within our expanding inherited cardiac conditions department. The successful candidate will work Monday to Friday 9 -5, with occasional cross cover at St Marys site.

Providing specialist nursing advice to support and facilitate high quality, individualised care to patients with inherited cardiac condition (ICC).

The successful candidate will have some knowledge and an interest of managing patients with or at risk of ICC, as well as working to identify and support at-risk family members and facilitate family screening. Early clinical review and will ensure individuals are cared for in the most appropriate setting utilising their clinical expertise, leadership, management and educational skills to ensure seamless care from diagnosis to end of life, training provided.

The post holder will provide long-term support to patients with inherited cardiac conditions and their families from the time of referral and throughout the care pathway.

They will work as a clinical expert offering advice, information and support to teams both in hospital and the community to ensure patients and families with or at risk of ICCs are managed appropriately.
