We have a great opportunity for a full-time band 6 nurse to work in Intensive Support Element of the Dementia & Older People’s Mental Health Team based in Worthing West Sussex. Intensive Support operates a to 9-5pm 7 days a week service including Bank Holidays.
The successful candidate will join a friendly, highly-skilled and enthusiastic multi professional team. We will take a keen interest in helping you to develop the skills important to this role and the post-holder will be encouraged to develop special interests and expertise in line with service and team objectives.
You will be part of a highly-skilled and supportive multi-disciplinary team. You will receive regular managerial and clinical supervision. This is challenging but rewarding work as we regularly receive positive feedback from our service users and carers. The post holder will be expected to travel across the geographical area (Adur, Arun & Worthing) covered by the team, the ability to travel to remote areas is essential.
This post offers the Trust’s £4,000 Incentive for band 5-7 Nurses & Practitioners, as well as a relocation package up to a maximum of £8,000 (subject to terms and conditions).

Job responsibilities
Clinical
To contribute to the management of a team caseload of clients with complex needs, using CPA principles, evidence based/client centred principles to assess, plan, implement and evaluate interventions, which will be delivered in the Dementia and Older Adults Mental Health Service.
To support/manage a team caseload and attend relevant MDT meetings
To regularly support and cover the duty rota
To provide specialist assessment and intervention as part of a multi-disciplinary approach, with specific responsibilities regarding the care pathway
To provide risk screening and assessment as well as risk management planning
To contribute to and facilitate evidence-based assessments and individual and group interventions
To contribute to the maintenance and development of the Service
To regularly share and supervise students on practice placement, and junior staff as appropriate
Knowledge/Experience
Essential
Post qualifying experience in the relevant care group
Experience of working alongside people with complex health problems in the designated care group
Experience of providing a range of clinical interventions to people in the designated care group with a variety of health problems
Experience of working consultatively with professionals
Experience of conducting clinical assessments including risk assessments
Experience of partnership working within and across various statutory and non-statutory teams and agencies
Experience of working within the CPA process
Understanding of clinical governance
Previous experience of working in the community
Qualifications
Essential
Registered Nurse with current registration [mental health/LD/Adult], appropriate to the job role
Completion of Mentorship Course/ENB equivalent
Evidence of continuous professional development
