Job summary
We are looking for an enthusiastic, innovative and engaging Band 5 NMC Registered Nurse, who has a distinct passion for the delivering high quality patient care within community services.
You will be joining our North Alliance LAP Service on a full-time basis to provide remote triage of calls and referrals made to the North community nursing teams.
As atriage staff nurse you will be expected to have professional and clinical knowledge of community nursing. Whilst not providing regular face to face patient contact you will need to maintain an up to date knowledge of the clinical aspects of community nursing in order to resolve clinical queries over the phone and to enable effective remote assessment and clinical triage. To ensure to a high standard of care is provided, occasional delivery of care to patients in a community setting in order to maintain up to date clinical experience.
You will be based at Weaver House, Cheadle. Due to the nature of this role shifts are 7-3pm,9-5pm or10-6pm covering 7 days a week.
Main duties of the job
Making appropriate decisions following comprehensive remote triage to ensure the safe management, and allocation visits where needed, in response to calls and referrals received by the community nursing service.
As part of this role you will also be required to:
Determine eligibility of patients referred to the service and collect additional information to support decision making. To transfer or redirect referrals to appropriate internal or external services as appropriate.
Liaise with patients; families; carers and other service providers to support clinical decision making to identify appropriate management of referrals and messages. This may include providing advice on self-management; transfer to other services or adding contacts for home visits or clinic appointments.
Liaise with nurse in charge of the appropriate nursing team to allocate urgent visits and to discuss potentially complex referrals.
To review all referrals and messages to the community nursing service, this will include an understanding of the following areas of care:
- Symptom management of deteriorating patients.
- Palliative and end of life care.
- Wound care.
- Bladder catheter and bowel care.
- Recognition of deteriorating patients and potentially life threatening conditions.
- Complex patients with co-morbidities.

Person Specification
- Experience of working as a Community Nurse.
- Supporting service users, families and carers in dealing with people with nursing needs
- Clinical knowledge relevant to the post.
