The post offers the successful candidate a chance of caseload management in a community setting .Working one to one with patients referred into the service the successful candidate will benefit from being part of a forward thinking dynamic team working towards the goals and outcome measures of the commissioning pack for Cardiac Rehabilitation.

- Two years post registration experience, 1 of which must be in CHD care
- Demonstrate leadership skills.
- Evidence of ongoing professional development
