Community Nurse – Learning Disabilities – Band 6

An exciting opportunity has arisen for experienced and highly motivated individual to join community city wide service within the Learning Disabilities Service.

You will be an experienced and flexible individual with excellent communication skills and a passion for multi disciplinary team working.

Main duties of the job

To work within the framework of a multi-disciplinary team to provide support and guidance to children and adults with learning disabilities and their carers.

To work within the framework of health facilitation (level one and two).

To co-ordinate a clinical service under the supervision of Senior Community Nurses.

Qualifications/training

Essential

  • Registered Nurse: Learning Disabilities or Mental Health
  • Post qualification training

Desirable

  • ENB 998 or equivalent

Experience

Essential

  • Significant post registration
  • Working with Multi-disciplinary Teams
  • Dealing with complex health needs
  • Working with Multi-agencies

Skills

Essential

  • Knowledge and understanding of assessment processes
  • Understanding diversity
  • Level 1 & 2 health facilitation
  • Understand legislation in relation to people with learning disabilities
  • Ability to undertake and understand audit tools
  • Knowledge of Care Programme Approach
  • Good IT skills
  • Good communication skills both written and verbal
