Job summary

The successful applicant will be an experienced cancer nurse with proven clinical skills and knowledge in the field of cancer and a commitment to prividing the highest standard of care to Breast Cancer patients.

We are looking for an individual with strong communication skills who has the ability to impart complex information to patients, relatives and carers.

Main Duties:

To be the key worker for adults with breast cancer under the medical and clinical oncologists and provide CNS service to the MDT clinics.

To participate and support the service development and adapting ways of delivering high quality care to ensure compliance with national guielines.

To provide expert nursing support before, during and after treatment. This will include outpatients, inpatients, telephone advice and triage.

To lead patient education and counselling at all stages of the pathway from entry into oncology service.

To support the development of nurse led clinics as the service requires for on treatment and follow-up for patients ensuring the patient and those close to them fully understand the treatment options, risks and side effects.

Establish contribute to stratified follow-up pathways for patients in accordance with national best practice guidance.

To take a lead role in the MDT clinics, participating in discussion and decision making, and ensuring the efficient implementation of clinical decisions and care pathways.

To work with the MDT and other CNS colleagues to establish and set measurable standards of care and ensure the delivery of a high-quality service for patients, using nursing audit and other measurement tools.

Lead on the delivery of timely holistic needs assessments (HNAs).

To ensure patients with complex needs are identified early

Liaise with referring teams both internal and external to UCLH

Develop key working relationships with other specialities such as palliative care for patients at end of life and/or those with complex symptoms.

Develop key working relationships with primary care including general practitioners and community palliative care teams. Develop knowledge base and clinical expertise in expected toxicities of routine and novel treatments.

Provide educational sessions and mentorship to junior nursing staff.

Attend and speak at network, regional and national meetings.

Attend team meetings with CNS and Breast MDT colleagues, in order to plan and evaluate the specialist nursing service and facilitate case discussion and support.

Attend relevant divisional and other meetings (including Cancer CNS community of practice) as agreed with manager
