Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare. Nursing vacancies are also open to Student Nurses and Newly Qualified Nurses.

The All Age Continuing Care service leads the assessment, case management and oversight of the NHS National Framework for Continuing Healthcare (CHC) and Funded Nursing Care (FNC) across Kent and Medway. We support individuals with complex or long-term health needs by developing, commissioning and reviewing personalised care packages in both community and care-home settings.

Working as part of a truly integrated system, our teams collaborate closely with the Local Authority, Primary Care Networks and wider partners to deliver safe, person-centred care.
