Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare. Nursing vacancies are also open to Student Nurses and Newly Qualified Nurses.
The All Age Continuing Care service leads the assessment, case management and oversight of the NHS National Framework for Continuing Healthcare (CHC) and Funded Nursing Care (FNC) across Kent and Medway. We support individuals with complex or long-term health needs by developing, commissioning and reviewing personalised care packages in both community and care-home settings.

Main duties of the job
As a Band 7 Senior Nurse Assessor, you will provide expert leadership within the All Age Continuing Care service. Key responsibilities include:
- Leading the CHC Nurse Assessor team through day-to-day supervision, mentoring and clinical guidance
- Delivering complex CHC assessments, including Checklists, Fast Track Assessments and Decision Support Tools
- Completing Mental Capacity Assessments, Best Interest Decisions and contributing to Court of Protection/DoLS processes
- Supporting the development of personalised care packages, including Personal Health Budgets
- Ensuring care packages are safe, effective and represent good value for money, while identifying cost-improvement opportunities
- Working collaboratively with the Local Authority throughout the assessment process and supporting the resolution of disputes
- Participating in the Local Resolution Process (appeals), including two-way discussions and panel involvement
- Providing expert advice on the National Framework, supporting its implementation, training and development across Kent and Medway
- Promoting excellent patient experience, adapting communication with kindness and compassion
- Overseeing team performance, ensuring high-quality, consistent and timely assessments
Qualifications, training and experience
Essential
- You’ll need to have a Level 6 Degree or equivalent qualification in relevant subject i.e. Nursing, Physiotherapy, Occupational Therapy or Speech and Language Therapy.
- Proven ability to work effectively within your area of practice, supported by relevant experience. As well as managing complex caseloads and supporting/ mentoring others.
- You will be a registered health care professional with the Nursing and Midwifery Council or Health and Care Professionals.
Knowledge and Skills
Essential
- You will need to be able to be able to use IT systems and databases.
- You will understand where to signpost clients and families to alternative services to support behaviour change and improve health.
- You will have knowledge of up to date clinical practice by using up- to- date skills and adhering to evidence-based procedures and clinical effectiveness frameworks. You will fully understand the role and remit of a health care worker.
Desirable
- Having an awareness of developments within the NHS would be an advantage as well as an understanding of National Service Frameworks and NICE guidance.

- A full, valid UK driving licence or a full driving licence issued by an EU country (not exchanged from a non-EU country) and the ability to travel independently are essential for this role.
